Church Grant Readiness Intake Form

This intake form is designed to help us understand your church's current organizational and financial readiness to receive grant funding.

MINIMUM ELIGIBILITY REQUIREMENTS

To be considered Grant Ready, churches should at minimum be able to provide:

• Employer Identification Number (EIN)

• Documentation of eligible 501(c)(3) tax-exempt status (preferred)

• Two years of Profit & Loss Statements

• Most recent Form 990, when applicable

Additional requirements may apply depending on the specific grant opportunity.

Completing this form does not guarantee funding. Your responses will help determine whether your church currently meets the minimum requirements for grant opportunities and identify areas where additional preparation or technical assistance may be needed.
